WE ARE ASKING ALL OF OUR PATIENTS TO
PRESENT A PHOTO ID AT THE
TIME OF ARRIVAL

**If the patient is a minor child, we ask that his or her parent or legal guardian provide a photo ID if one is not available for the child.

Effective May 1, 2009, Cape Fear Orthopaedic Clinic must comply with The Fair and Accurate Credit Transactions Act of 2003 (FACTA) as amended in October 2007 to include Red Flag and Address Requirements (Red Flag).  In order to comply with FACTA and the "Red Flag" rule, we are required to implement an Identity Theft Prevention Program effective May 1, 2009.  The Red Flag rules are designed to protect consumers from identity theft by requiring certain businesses, including medical facilities, to recognize activities that indicate possible identity theft and to take measures to verify the identity of current and potential clients.
