Appointments

Our patients are seen by appointment only. Appointment times vary by provider, but our office hours are the same at all locations.  We ask that you please request appointments as far in advance as possible so that we can better accommodate your specific needs. If you are unable to keep a scheduled appointment for any reason, please give our office at least 24 hours notice so that we can offer that time slot to another patient. Getting Ready for Your Visit

If you are a new patient, when you arrive for your appointment we will ask you to complete the following forms:

For your convenience, the forms can be printed from our website if you would like to complete them in the convenience of your home. Just remember to bring them with you to your appointment. It is important that you fill in all of the requested information completely and accurately. In addition to the required forms, you’ll need to bring the following information with you to the appointment:

  • Valid government issued photo ID (i.e. drivers license, military ID, etc.)
  • Your current health insurance card(s)
  • A list of your medications including the dosage and any allergies
  • Office notes and X-ray/MRI films from your referring provider

If you are an established patient (seen in the past 3 years) we may request that you update registration and medical history forms, if necessary. Please be sure to notify our receptionist when you check-in if there are any changes to your name, address, telephone number or insurance information.


Treating Minor Children

All minor children under the age of 18 must have at least one parent or legal guardian present at the time of the first appointment. This includes any previous patients being seen for a NEW injury or problem.

  • Minors age 15-17 may attend follow up appointments without a parent or legal guardian only if we have a signed Authorization to Treat a Minor Child form on file.
  • Minors age 14 and under must have a parent or legal guardian present at all appointments unless we have a signed Authorization to Treat a Minor Child form on file designating an authorized adult to accompany the child. Children under the age of 15 may not be seen without an authorized adult present.

Billing and Insurance

Our clinic participates with most major insurance plans which includes but is not limited to the following:

  • Medicare
  • Medicaid of NC
  • Tricare
  • Doctor’s Direct Healthcare
  • Medcost
  • BCBS of NC
  • Cigna
  • United Healthcare
  • Wellpath
  • State Employees Health Plan

Copayments and all other patient balances must be collected at the time of service. Payment for services not covered by your health plan is your responsibility. If you are unable to pay large balances all at once, our staff will be happy to assist you with payment arrangements. We accept payment by cash, check, debit card, Visa, MasterCard or Discover. For your convenience, you may also pay your bill online through our website.

If we do not participate with your insurance plan, we will file your claim as a courtesy to you; however, payment is due in full at the time of service. If you have questions about whether we participate with your health insurance plan, please contact our billing office at (910) 484-2171 ext. 509.

If your health plan requires authorization to be treated, please bring it with you to your appointment. If you do not have the necessary referral, we may need to reschedule your appointment.


Workers' Compensation

Treatment for injuries due to a work accident must be authorized by your employers’ workers compensation carrier. We will need contact information for the claims adjuster and case manager in order to file claims and update your work status. Should your case become a legal issue and you retain and attorney, please be sure that we have the attorney contact information and signed releases from you in order to communicate with them.


Third Party Liability

Because our clinic cannot wait for third party liability claims to be settled, we do not accept third party liability. Payment for services resulting from automobile accidents or other accidents is the responsibility of the patient and is expected at the time of service.


Emergencies

If you are an established patient and you have an orthopaedic emergency when the office is closed, you may contact our answering service at (800) 241-5513. Emergency and urgent phone calls will be directed to the physician on call and will be handled as soon as possible.


Prescriptions and Refills

New prescriptions and refills are only issued during normal business hours. You can request a prescription by calling (910) 484-2171 ext. 547 or you may request a prescription online through our website. Because your doctor is only in clinic 2-3 days per week, we ask that you please allow us a minimum of 72 hours to process your medication request. Please do not wait until you have run out of medication to request a refill. Certain narcotic prescriptions must be written by law and may NOT be called into your pharmacy. Written prescriptions can be picked up any time during regular business hours. Please be prepared to show a photo ID when picking up prescriptions.


Medical Records

A copy of your medical records and/or x-ray films is available upon your written request. You should complete an Authorization to Release Protected Health Information form to request such copies. We do charge a fee for medical record and x-ray copies and may ask for prepayment of the fee. Please allow us 5-7 business days to complete your request. Completion of Forms

Forms that need to be completed by your provider may be turned in to the Medical Secretary or at the front desk. We do charge a nominal fee (per form) to complete forms and require all fees to be paid at the time the form is dropped off for completion. Please allow us 48-72 hours to complete your form.
