Getting Ready for Your Visit

If you are a new patient, when you arrive for your appointment we will ask you to complete the following forms:

For Children (ages 17 and under):

For your convenience, the forms can be printed from our Web site if you would like to complete them in the convenience of your home. Just remember to bring them with you to your appointment. It is important that you fill in all of the requested information completely and accurately. In addition to the required forms, you’ll need to bring the following information with you to the appointment:

  • Valid government issued photo ID (i.e. drivers license, military ID, etc.)
  • Your current health insurance card(s)
  • A list of your medications including the dosage and any allergies
  • Office notes and X-ray/MRI films from your referring provider

If you are an established patient (seen in the past 3 years) we may request that you update registration and medical history forms, if necessary. Please be sure to notify our receptionist when you check-in if there are any changes to your name, address, telephone number, or insurance information.

Treating Minor Children

All minor children under the age of 18 must have at least one parent or legal guardian present at the time of the first appointment. This includes any previous patients being seen for a NEW injury or problem.

  • Minors age 15-17 may attend follow-up appointments without a parent or legal guardian only if we have a signed Authorization to Treat a Minor Child form on file.
  • Minors age 14 and under must have a parent or legal guardian present at all appointments unless we have a signed Authorization to Treat a Minor Child form on file designating an authorized adult to accompany the child. Children under the age of 15 may not be seen without an authorized adult present.
